Patient Health Summary form must contain the following information:

Under the PHIPAct. 2004 of Ontario, Kelly tries her best to ensure PHIPA/HIPAA compliances are met.
Billing Scheme:
In short, ALL ACUPUNCTURE (INSURANCE eligible) appointment must be presented in person at Step Up Yonge & Bloor. Step Up Clinic will handle all billings. Operates under a different set of policies established by the clinic.
**Exceptions: Lifestyle / Diet / Chronic Illness Management / Herbal Consultation with TCM Diagnosis ONLINE are eligible for insurance claim. They will be submitted under TCM Diagnosis. Separate receipt will be sent to your email.
These are the ONLY services offered online eligible for insurance claim.
When hypnosis / hypnotherapy is conducted online, there is no insurance claim for these items.
Acupuncture / TCM - There is no discount policy as per professional college standard.
Legal Regulations:
Acupuncture / TCM is regulated under TCM Act. 2006.
Acupuncture / TCM is regulated under RPHA Act. 1991.
Regulated R.Ac., R.TCMP. follows Clean Needle Technique (CNT®) and infection control protocols.
Kelly has completed IPAC (Public Health Ontario) in 2017 & 2022.
Hospital work clearance Synergy Gateway verified in 2017 & 2021. (Expired)
Ministry of Labour Training completed in 2017.

Exclusive: All Patients visited in person from 2017-2023 - house call visits / occasional - Kelly has no place to work uptown but Kelly doesn't want to abandon patients from before. House calls available. She may borrow someone's home/others' clinic if necessary. Kelly DOES NOT accept new patients privately. Kelly DOES NOT answer phone calls. Please use SMS or WhatsApp as she expects no new patients contacting her privately.
